I give permission for my child to attend Block Party 2026. I understand photos may be taken during the event and may be used by Family Church.
I understand that some of the associated risks include, but are not limited to sunburn, dehydration, insect bites, sports related injuries, and potentially more serious injuries or drowning resulting from participation in various watersport activities.
My student has permission to be released to the guardian who dropped them off.
